Appointment Scheduling Details

 

• Well Visits - To schedule a well visit or general office visit, we recommend calling several weeks in advance to obtain the most convenient appointment for your schedule.

 

• Sick Visits - Children who are ill are seen the same day. We recommend calling in early in the day to arrange to see your child.

 

• Cancellation/Re-scheduling - We request 24 hour cancelling/request to reschedule or last minute cancellation charges will apply.If you do not have a scheduled appointment after 5pm or on the weekends please call our office to ensure we are open for the evening and             weekend posted hours. There are occasions that we are not open the full evening or weekend hours.

 

    Please be advised prior to your appointment you will be required to pay your copay and any outstanding balance on your account. Failure to            provide your copayment or balance payment at the time of your visit can result in your appointment being rescheduled to a different time or          day. All copays and balances are due at the time of your visit.  This is our office policy to collect all copays and balances prior to being seen.

 

   Our office will complete all forms requests within 5-7 business days. All forms regardless of the type of form (medication form, camp form,           school form, sports form etc..) will result in a $5.00 per form fee. All forms must be picked up at the office. We do not mail or fax any forms of       any kind to anyone.
